Institutional Financial Aid Forms

The following documents are for the 2008-2009 academic year. The academic year is summer 2008, fall 2008, and spring 2009. The documents are available in .pdf, portable document format, which can be opened using Adobe Acrobat Reader. If you do not currently have this software, it is available for free here.
Every student must complete a Montana Tech Data Form every academic year. Please open and print Pdf/Data Form 2008-2009_regular_.pdf or, if you will be a graduate student, Pdf/Data Form 2008-2009 _graduate_.pdf. You can also request one be sent to you from our office or you can pick one up from our office. Please send the completed form to the Financial Aid Office.

If you are considered a dependent student by Federal Regulations and have been selected for a process called verification, please open and then print Pdf/0809VerWkshtDependent.pdf and Pdf/worksheet ABC.pdf. Please send the completed worksheet and copies of 2007 Federal Income Tax Return and w-2s for both student and parents.

If you are an independent student and have been selected for a process called verification, please open and then print Pdf/0809VerWkshtIndepend.pdf and Pdf/worksheet ABC.pdf. Please return the completed form and copies of your and/or spouses 2007 Federal Income Tax Return and w-2s.
